Fall Sports Tryouts
The tentative first day of tryouts for fall sports is July 29th. All athletes must be fully cleared to participate in tryouts. Athletic Clearance should be submitted no less than 48 business hours before tryouts.
Required Medical Documents
Student-athletes must have the following documents on file:
ECG (EL1 Form)
- ECGs are valid for 4 years.
- Incoming 9th-grade students may be subject to additional ECG requirements under current FHSAA guidelines.
Physical (EL2 Form)
- Physicals are valid for 1 calendar year.
- Page 4 of the EL2 form is the primary page required for athletic clearance.
- The physician’s signature must be either wet ink or a provider stamp.
- The student-athlete information at the top of the page must be completed.
- The parent/guardian signature section must be completed.
- The physician information section must be completed in full.
ECG Submission Tips
If an ECG is completed through Who We Play For, a hospital, or another healthcare provider and is electronically signed by a licensed physician, families must also upload documentation showing the ECG was reviewed and determined to be normal, along with the EL1 form.
Acceptable documentation includes:
- A copy of the normal results from the patient’s health record
- A confirmation email from the healthcare provider
Submitting only the ECG tracing or readings will not be accepted.
NFHS Courses
The following NFHS courses must be completed every school year by all student-athletes:
- Concussion in Sports
- Heat Illness Prevention
- Sudden Cardiac Arrest
- Sportsmanship
In addition, football players must complete:
- Football Tackling
Certificates of completion must be uploaded to AthleticClearance.com.
Additional Documents Required
Families must also upload:
- A copy of the student-athlete’s birth certificate
- A copy of the student-athlete’s current medical insurance card
- A copy of the parent/guardian’s photo ID
