Clearance Instructions – Cornerstone Charter

Athletic Clearance is the online portal OCPS uses to collect all physical and ECG paperwork.

Hard copies will NOT be accepted.

All students must submit a new clearance every school year. A physical is valid for a full calendar year and must be resubmitted in the new years clearance if it is still valid. Do this by selecting "Choose Existing File" on the File upload page.

Any documentation submitted during the 2024-2025 school years must be resubmitted in the new clearance. Do this by selecting "Choose Existing File" on the file upload page.

NFHS Required Courses

All student-athletes must complete 3 courses offered by the NFHS every year. These courses are:

Sudden Cardiac Arrest
Heat Illness Prevention
Concussion in Sports

Any students playing tackle football will also be required to complete the Football Tackling course.

Athletic Fees

Athletic fees are required to be paid to participate on a Cornerstone Athletics Team.

If you quit or are removed from a team for discipline issues you will not receive your athletic fee back.

Student-athletes/parents can make a payment by going to this link to learn how to access the AgaveX portal. Payment is due before the athlete will be able to participate in games.

Clearance Deadline

Clearances must be submitted 48 business hours before the first day of conditioning/tryouts in order to be eligible on the first day.

Eligibility Requirements

Students participating in any CCA team sport must:

Have an unweighted cumulative GPA of 2.00 or higher.
Submit all paperwork digitally through the online athletic clearance site.
Pay their athletic fees.

To Create a New Clearance

Go to: http://athleticclearance.fhsaahome.org/

First time users: Create an account. Parents/guardians must register with a valid email address. Email must be the parent/guardian’s email address and one that is checked.
Sign in using the email address that you registered with.
Select "Start Clearance Here" to start the process.
Select:
- School Year: 2025-2026
- School: Cornerstone Charter High School (Belle Isle)
- Sport/s: Select ALL the sports/activities that your child plans on trying out for.
  - Students are only eligible for the sports/activities selected at this step.
  - Failure to select a sport/activity will result in ineligibility for a sport/activity that is not added to their account.
Complete all required fields:
- Student Information (Enter the grade the athlete will be for the 25-26 school year)
- Parent/Guardian Information
- Medical History
- Signature Forms (Names must be exact. Case sensitive)
- Upload required files:
  - FHSAA EL2 Physical form
  - OCPS Cardiology Report
  - ECG Results email
  - NFHS course certifications for Sudden Cardiac Arrest, Concussion for Students, Heat-Illness Prevention, and (for students playing football) Football Tackling
  - Medical Insurance Card
  - Parent ID Card
  - Student Birth Certificate
Students that were not enrolled at Cornerstone on their first day of 9th grade are also required to complete a GA04 form.
Once you reach the Confirmation message you have completed the online registration process.

This does NOT mean the student is cleared.

Hitting submit means your files will be electronically filed with your school's athletic department for review. When the student has been cleared for participation, an email notification will be sent. This may take up to 48 hours.
